The following forms have been custom designed to offer all Travis County citizens the opportunity to save time and a trip to the office by accessing them all online. Please follow each documents instructions carefully and remit all completed applications to the correct office as outlined on the form itself.

All forms were structured in a PDF format which allows them to be downloaded using Adobe Acrobat Reader 4.0 or later. Most of these forms have been made available in both English and Spanish (Español). If you are unable to open or access these forms for any reason, please call our office at (512) 854-9473 so that we may attempt to fax or mail them to you.
